What You Need to Apply
- A completed DS-11 form
- One 2”x2” passport photo
- Passport photos are available at the Elizabeth and Westfield offices for $10.
- Evidence of child’s U.S. citizenship
- Evidence of relationship between child and parent(s)/guardian(s)
- Parent(s) or guardian(s) identification
- Provide parental consent
- Pay applicable fee

Renewal of Passport for Children
To renew a passport for a child (15 or younger), the above requirements are the same as well as the submission of the old passport.
Please Note: A passport for a child cannot be renewed by mail.
