Passport Appointment Checklist

How to get a passport

We are processing passport applications by appointment onlyThe US Department of State’s estimated processing time is 6 to 8 weeks for routine service and 2 to 3  weeks for expedited service. We strongly encourage you to check your passport now and plan accordingly.


All applicants MUST appear in person for the appointment. Please be on time for your appointment. ALL PASSPORT APPLICATIONS MUST BE COMPLETED, IN BLACK INK, PRIOR TO YOUR ARRIVAL. You can visit the Department of State website to obtain the applications. You also have the option to pick up the applications at our office BEFORE your appointment but please allow for plenty of time to complete your application in your car if you choose to pick up the applications before your scheduled appointment.


The following items are essential to obtain a passport:

  1. Proof of U.S. Citizenship
    • Applicants may present one of the following:
      • An original, US birth certificate
        1. Issued by the city, county, or state of birth
        2. Lists your full name, date of birth and place of birth
        3. Lists your parent(s) full names
        4. Has the date filed with registrar’s office (must be within one year of birth)
        5. Has the registrar’s signature
        6. Has the seal of the issuing authority
      • Most recent, undamaged US passport or, if lost, any previous passport
      • Certificate of Naturalization or Citizenship (Original)
      • An original Consular Report of Birth Abroad
      • Please contact our office for more information concerning claiming citizenship through US Citizen or Naturalized Parents as well as adoption from a foreign country.

  2. Proof of Identification
    • Applicants must present at least one of the following:
      • Valid, undamaged U.S. passport book or passport card
      • In-state, fully valid driver’s license with photo
      • Certificate of Naturalization or Citizenship
      • Government employee ID (city, county, state, or federal)
      • U.S. military or military dependent ID
      • Valid foreign passport
      • Matricula Consular (Mexican Consular ID only)
      • U.S. Permanent Resident Card (Green Card)
      • Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
      • In-state, fully-valid non-driver ID with photo
  3. Passport Photo
    • We offer passport photos for $10
    • If you supply your own photo, if not from us
      • Submit a color photo, taken in last 6 months
      • Use a clear image of your face. Do not use filters commonly used on social media
      • Have someone else take your photo. No selfies
      • Take off your eyeglasses for your photo
      • Use a plain white or off-white background
      • Do not wear a uniform or camouflage, of any color
  1. Children (15 and younger)
    • You must submit documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport.
      The following original documents may be used to show parental relationship:

    • You will need to know the child’s Social Security number for the application.
    • The parent(s) or guardian(s) must present one valid form of ID to the acceptance agent
    • Named parent(s)/guardian(s) must authorize the issuance of the child’s passport
      • If one parent/guardian cannot go with the child to apply for the passport, they can consent by completing Form DS-3053 “Statement of Consent. “You must submit the completed form with the child’s passport application. The parent that cannot go with the child must:
        1. Sign and date Form DS-3053 in the presence of a certified notary public, and
        2. Submit a photocopy of the front and back side of the ID that they present to the notary public with Form DS-3053.
        3. Please note: In certain countries, a DS-3053 must be notarized at a U.S. embassy or consulate and cannot be notarized by a local notary public. Go to the U.S. embassy or consulate webpage for more information.
  2. Children (16 or 17)
    • One parent may need to present evidence that they are the Parent/Guardian of the child applying for a Passport.  The parent’s signature or a check signed by the parent is required.
