The form is not necessary.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Maximum Contribution Amounts for 2025:\n<ul class=\"wp-block-list\">\n<li>Medical FSA Accounts: $3,400<\/li>\n\n\n\n<li>Dependent Care FSA: $7,500<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Delta Flagship Ending<\/strong>\n<ul class=\"wp-block-list\">\n<li>The Delta Flagship plan will be ending effective December 31, 2025. Temporary cards can also be printed from this account while the cards are being mailed out to the home address. If new cards are ordered, please allow up to 10 business days for them to arrive.<\/li>\n\n\n\n<li><strong>EPO, HSA, Direct Access, and PPO Plan Members &#8211;<\/strong>&nbsp;Receive 2 cards in total: 1 card for medical coverage and 1 card for prescription coverage.
