Jersey Assistance for Community Caregiving (JACC) is a transformative program that aims to enhance the lives of unpaid caregivers and the individuals they care for. One of the key pillars of JACC is its provision of in-home services. Certified aides offer invaluable assistance with activities of daily living, such as bathing, grooming, and meal preparation. This hands-on support not only lightens the caregiving load but also ensures that individuals in need receive the compassionate care they deserve. By supplementing the assistance provided by caregivers, JACC enables seniors and individuals with disabilities to continue living independently in the comfort of their own homes.
Qualified individuals may have the opportunity to hire their own family, friends, or neighbors to provide the needed care. A case manager will work closely with the individual to develop a plan of care that best meets their needs. Eligibility Requirements
To qualify for JACC, the person receiving care must meet the following criteria:
- Be a Union County Resident
- At least 60 years old
- Be a US citizen or qualified alien
- Must not be eligible for MLTSS or other NJ Family Care
- Assets at or below $40,000 (Individuals) or $60,000 (Couples)
- Not live in Assisted Living Facility or Residential Care Facility
- Not participate in SRCP or NJ Alzheimer’s Adult Day Services
