Notice is hereby given that the County of Union, Division on Aging, will receive proposals on

Thursday, January 17, 2019 at 11:00 A.M in the Union County Administration Building, 10 Elizabethtown Plaza, Freeholders Meeting Room, 6th Floor, Elizabeth, NJ, for the provision of:


Coordinated Community Home Care and Statewide Respite Services

January 1, 2019 through December 31, 2021


Proposals are being solicited through a fair and open process in accordance with N.J.S.A. 19:44A-20.4.


Requests for Proposals may be obtained beginning on December 13, 2018 by:

  1. Downloading the proposal from the County’s Webpage (ucnj.org);
  2. Directly by visiting (http://ucnj.org/vendor-opportunities/rfqs-and-rfps/current/);
  3. A hard copy may be picked up at the Division on Aging, Union County Administration Building, 4th Floor, 10 Elizabethtown Plaza, Elizabeth, New Jersey during the hours of 9:30 a.m. to 4:30 p.m. beginning on December 13, 2018; or
  4. Proposals may be mailed upon request by calling the Division on Aging at (908) 527-4867


All proposals must meet the targeting policy of Union County Division on Aging, which prioritizes persons 60 years of age and older with the greatest social and economic needs.

Services to be funded include:

1.Coordinated Community Care for home care services for residents of Union County, 60 years of age or older, whose income falls within specific guidelines and/or are receiving Social Security Disability. Services include: certified home health aide and visiting nurse services. Total funding available $100,000.


  1. Statewide Respite Care Program to provide respite to unpaid caregivers 18 years or older, caring for seniors or disabled persons that meet established income and resource guidelines. Services include: medical adult day care (Medicaid rate), social adult day care ($45/hr), homemaker/housekeeping ($17/hr), certified home health aide ($17/hr or Medicaid rate), companion ($12/hr), Assisted Living respite, and In-Patient respite. Total funding available $261,425.


Supplemental services under the Statewide Respite Care Program include (dependent solely on cost share collections therefore funding is not guaranteed):

  1. Alzheimer’s Caregiver Conference- Provision of an educational conference/workshop for caregivers which provides practical information (medical/legal/behavioral) for those caring for individuals with Alzheimer’s or other dementia related diseases (Maximum funds $6,000)


In an effort to assist candidates intending to apply for these funds, Interested Proposers are strongly encouraged to attend the Technical Assistance Meeting to be held on January 8, 2019 9:30a.m -11:30a.m. at the County Administration Building, 4th fl. Conference Room

Proposals must be made in the form required by the specifications and One (1) Original Paper hard copy, Four (4) copies, and One (1) PDF electronic copy on a CD or Thumb Drive must be delivered prior to the stated time for the receipt of proposals to:

All information requested in the Request for Proposals must be provided or the proposal may be disqualified.  Proposals MUST BE SEALED and labeled “PROPOSALS – DIVISION ON AGING-SERVICES FOR SENIORS” prior to the stated time for the receipt of proposals and received no later than 11:00 AM on January 17, 2019.  The Board of Chosen Freeholders reserves the right to reject any and all proposals, to waive any informality in the RFP process, and to accept any proposals which, in their judgment, are most advantageous, price and other factors considered, and will best serve the interest of Union County.


Proposers are required to comply with the requirements of N.J.S.A. 10:5-31 et seq. and N.J.A.C. 17:27.
