Proposals are being solicited through a fair and open process in accordance with N.J.S.A. 19:44A-20.4.
***Proposal delivery is strongly encouraged to be made by Commercial Mail Carrier (USPS, FEDEX, UPS) so that tracking information is confirmed. Hand deliveries to the Administration Building will be received but without a receipt of confirmation.

Other Insurance Provisions The insurance policies are to contain, or be endorsed to contain, the following provisions: Primary Coverage – For any claims related to this contract, the Consultant’s insurance coverage shall be primary and non-contributory as respects the County of Union, its officers, officials, employees, agents, and volunteers. Acceptability of Insurers – Insurance is to be placed with insurers authorized to conduct business in the state with a current A.M. Best’s rating of no less than A:VII, unless otherwise acceptable to the County of Union. Verification of Coverage – Consultant shall furnish the County of Union with Certificates of Insurance and amendatory endorsements before work begins. Claims Made Policies – If any coverage required is written on a claims-made coverage form: The retroactive date must be shown, and this date must be before the execution date of the contract or the beginning of contract work. Insurance must be maintained and evidence of insurance must be provided for at least three (3) years after completion of contract work. If coverage is cancelled or non-renewed, and not replaced with another claims-made policy form with a retroactive date prior to the contract effective, or start of work date, the Consultant must purchase extended reporting period coverage for a minimum of three (3) years after completion of contract work. Subcontractors -Consultant shall require and verify that all subcontractors maintain insurance meeting all the requirements stated herein, and Consultant shall ensure that County of Union is an additional insured on insurance required from subcontractors. Special Risks or Circumstances – The County of Union reserves the right to modify these requirements, including limits, based on the nature of the risk, prior experience, insurer, coverage, or other special circumstances. Below Language MUST be included on the ACORD Insurance Form in the Description Box while the contract is in effect: The County of Union, its officers, officials, employees, agents, and volunteers are included as additional insured as required by contract with the County of Union. Certificate Holder is: County of Union All medical staff MUST submit a Business Registration Certificate (BRC) from the State of New Jersey if the value of their contract exceeds $5,400.00 during a 12-month period. Proof of business registration shall be: • A copy of a Business Registration Certificate issued by the Department of Treasury, Division of Revenue; or In addition, all medical staff MUST comply with producing Affirmative Action evidence if the value of their contract exceeds $36,000.00 during a 12-month period. General Requirements of P.L. 1975, c. 127 – All successful vendors must submit one of the following: • A photocopy of your Federal Letter of Affirmative Action Plan Approval; If the medical staff does not produce the above once the applicable thresholds are reached, the County cannot issue payment and this could delay the payment process. Proposers provide services as a consulting and/or attending practitioners.
