Probating a Will?
Things You Will Need:
- ORIGINAL Death Certificate.
- The death certificate must have a raised seal and it must show the decedents county of residence as UNION COUNTY.
- ORIGINAL Last Will and Testament and all Codicils.
- List of Next of Kin.
- This list must include full names and address.
- Who is considered next of kin

Filing for an Administration or Affidavit?
Things You Will Need:
- ORIGINAL Death Certificate. The death certificate must have a raised seal and it must show the decedents county of residence as UNION COUNTY.
- List of Assets. This needs to be a detailed list with assets in the decedents name only.
- List of Next of Kin. This list must include full names and address. Who is considered next of kin

Filing a Minor Guardianship?
Things You Will Need:
- Minors ORIGINAL Birth Certificate.
- Minors ORIGINAL Social Security Card.
- Parent/Guardianship’s Photo ID Card.
- Information on where the funds for the minor are coming from.
- Minor Guardianship Information Sheet
- If the minor is over 14 years old and one or more parents are deceased, the minor must accompany the applicant for the appointment.
Filing a Guardianship for an Incapacitated Person?
Things You Will Need:
- COMPLETED Verified Complaint Application.
- Person & Property: How to Apply for Guardianship of the Person and Estate (Property) of an Individual Eligible for Services from the Division of Developmental Disabilities (DDD)
- Person Only: How to Apply for Guardianship of the Person of an Individual Eligible for Services from the Division of Developmental Disabilities (DDD)
- Certificate of Criminal and Civil Judgement
- $200 filing fee.
