Waiving Benefits

Employees electing to waive any or all insurance plans need to complete a Health Benefits Waiver Form to terminate insurance coverage.

This form will take the place of the waiver statements located on previous insurance enrollment forms.

DO NOT submit an enrollment form for any benefit being waived stating that the benefit is being declined. Only this form is needed for this type of change.
