NJ FamilyCare is New Jersey’s publicly funded health insurance program, providing eligible individuals and families with access to affordable health coverage. Coverage may include doctor visits, hospital services, prescriptions, preventive care, behavioral health services, and other medically necessary services.
The Union County Division of Social Services assists eligible residents with NJ FamilyCare programs, including Medicaid coverage for individuals who are aged, blind, or disabled.

NJ FamilyCare Application Checklist
A friendly guide to help you get ready.
1. Before You Apply
Gather this information before you begin:
- The NJ address where you live and an address where you get your mail, if different from where you live.
- Names and birthdates of everyone in your household, and the due date of pregnant household members.
- A Social Security Number for everyone in your household, if they have one. You can still apply if you don’t have an SSN.
- U.S. Citizenship or immigration status of everyone asking for NJ FamilyCare coverage.
- Immigration status can include Lawful Permanent Resident, refugee, visa, etc.
- You will need to include information from the immigration document.
- Income and tax filing status of each person in your household.
- If employed:
- Employer name and address.
- Start month and year.
- Gross income (income before taxes are taken out).
- Know whether they are paid every week, once every two weeks, twice a month, or once a month.
- Other income, if received, such as SSI, unemployment, etc.
- Choose a Health Plan. Find out what plan your doctors accept.
2. Applying & Following Up
Simple steps that can help your application:
- Consider creating an online NJ FamilyCare account. You will need an email address to do this.
Benefits of an online account include:
- Access to an electronic copy of your application with NJ FamilyCare contact information.
- The ability to check your application status.
- The ability to upload documents instead of mailing or faxing them.
Be sure to:
- Fill out the application completely and correctly.
- Open all mail and respond quickly to any NJ FamilyCare letters requesting information.
You may be asked to provide proof of:
- Address.
- Identity, including IDs, birth certificates, residency documents, and passports.
- U.S. Citizenship or immigration status.
- Income, including profit and loss statements for people who are self-employed.
Call the NJ FamilyCare Eligibility Determining Agency listed on your Confirmation Page with any questions or to check your application status.
3. If You Are Approved
What to expect after approval:
- You should receive a plastic Health Benefits Identification Card within 1–2 weeks after your application has been approved.
- If you had NJ FamilyCare in the past, your old card might be reactivated.
- Questions or need a new card? Call 1-877-414-9251.
- Your Health Plan card and information will arrive about a week before that coverage starts.
4. Keep Your Coverage
Important actions after you are enrolled:
Tell the NJ FamilyCare Eligibility Determining Agency if there are any changes in your situation, including:
- Your address or phone number.
- Your income or employment status.
- The number of people living in your household, including pregnancy and birth.
- Marriage or divorce.
Also remember to:
- Open all mail from NJ FamilyCare.
- Renew your coverage every year!

NJ FamilyCare is publicly-funded health care coverage for New Jersey residents. It provides quality health care at little or no cost to its members.
What Is Covered?
NJ FamilyCare offers full health care coverage through health plans contracted by New Jersey. NJ FamilyCare covers most health care needs including:
• doctor visits • x-rays • eyeglasses • autism services • prescriptions • hospitalization • mental health services •community doula services • lab tests • dental • specialist visits • help with personal care needs
Who Is Eligible?
Children, adults, and pregnant women can qualify for NJ FamilyCare based on their family size and income.
The income limit for children to qualify is higher than the income limit for adults.
The federal government has changed the rules around who is eligible for NJ FamilyCare starting Fall 2026. These rule changes are part of the “One Big Beautiful Bill Act” that was signed into law by President Trump in July 2025. These new rules do not apply to everyone. NJ FamilyCare will contact you by Fall 2026 with more information if these changes apply to you. In the meantime, you should keep using your benefits.
Beginning October 1, 2026:
Some non-citizen immigrants may no longer qualify for NJ FamilyCare.
Beginning January 1, 2027:
- Certain adults aged 19 to 64 will have to work, volunteer, or go to school to keep their coverage or qualify for NJ FamilyCare. This is called “community engagement.” These members must also renew their coverage every 6 months (instead of every 12 months).
- Other adults aged 19 to 64 may be exempt from the requirement to work, volunteer, or go to school, but they may still have to renew their coverage every 6 months.

In determining whether a person is financially eligible for NJ FamilyCare ABD Programs, the applicant’s financial eligibility will be considered along with his or her spouse or parents. Use the ABD Checklist to see what you may need in order to apply.
