Appointments are strongly recommended. Those under 18 years old must be accompanied by a parent or legal guardian. Identification must be presented for both the parent/legal guardian and the child receiving the vaccination. Those under 18 years old are required to fill out a consent form. This form is also provided on-site.

Booster Guidance
Moderna and Pfizer (mRNA Vaccines)
- Everyone 5+ is eligible for the upgraded booster.
- Bivalent Boosters are recommended two months after their primary series and/or their last booster.
- Pfizer Bivalent Booster – ages 5+
- Moderna Bivalent Booster- ages 6+

Novavax Monovalent
- Novavax monovalent COVID-19 boosters are available for adults 12+ if they have completed primary series vaccination but have not previously received a COVID-19 booster—and if they cannot or will not receive an updated mRNA booster.



Booster Guidance for Infants effective December 20th
Infant Moderna Bivalent Booster
- For those 6 months to 5 years old
- 2 months after their primary series (2 doses)
- Only for those who completed Moderna primary series
Infant Pfizer Bivalent Booster
- For those who turned from 4 years to 5 years old
- 1 dose Infant, 1 Pediatric, 1 infant booster – 2 months after last dose.
Infant Pfizer Bivalent Primary
- For those 6 months to 4 years who have only received the first 2 of the 3 total in the primary series.
- This replaces the Pfizer infant 3rd dose.

Age Requirements for Vaccinations

Providing forms of identification are preferred, but not required for both the parent/legal guardian and the child receiving the vaccination. Please bring your vaccination cards if you are receiving your second, third or booster shots.

The service is available to all home bound Union County residents free of charge, as part of the Union County Commissioner Board’s efforts to ensure that COVID-19 vaccines reach every household in the community as quickly as possible. There is no income eligibility cutoff.
The home vaccination service is offered by the Union County Commissioner Board.
