• Uninsured, underinsured, or covered by Medicaid Plan A.
• Union County Resident

  • Parent’s Photo ID
  • Proof of Address in Union County (ONLY ONE REQUIRED)

    • Notarized letter from the person whom you are living with and one of their utility bills.
